ADULT REHABILITATION HOME AND COMMUNITY PHYSIOTHERAPY

Adult Rehabilitation Home Visits

Our Adult Rehabilitation Home Visit service offers professional physiotherapy support in the comfort and familiarity of your own home. Led by Holly Cummings, an experienced physiotherapist with over 18 years of NHS experience, this popular and growing service helps you or a loved one regain strength, confidence, and independence after illness, injury, or time in hospital.

Why Choose a Physiotherapy Home Visit?

Sometimes it’s not easy — or even possible — to attend a clinic. You may have recently come home from hospital, experienced a fall, or simply find that travelling feels like too much right now. That’s where home visits can make all the difference.

A physiotherapy home visit allows us to assess and treat you where it matters most — in your own environment. We can help you:

  • Regain strength and balance after illness or a hospital stay

  • Recover confidence and mobility after a fall or period of deconditioning

  • Rebuild independence with tailored exercises and movement practice

  • Prevent future falls or injuries through education and training

  • Adapt your environment for safety and ease of movement

Every visit is personalised to your situation and goals, supporting you to progress at a pace that feels right for you.

Recovery After Hospital

Many people find that after a stay in hospital — whether for surgery, illness, or another medical reason — they feel weaker, more tired, or less confident in their movement. This is completely normal, and with the right support, you can rebuild your strength and independence quickly and safely.

Holly’s home visits are ideal for post-hospital rehabilitation. She can:

  • Help you regain mobility and move safely around your home

  • Create a personalised exercise plan to rebuild stamina and confidence

  • Support with walking aids or equipment to make daily life easier

  • Offer advice for family members or carers to help your recovery

  • Coordinate with your wider healthcare team if needed

This type of physiotherapy bridges the gap between hospital discharge and feeling fully confident at home again — ensuring your recovery continues smoothly.

What to Expect During a Visit

Holly will start by getting to know you — understanding your current abilities, challenges, and what you’d like to achieve. She’ll then carry out a thorough assessment and design a clear, achievable rehabilitation plan to help you work towards your goals.

Visits may include:

  • Gentle, progressive exercise programs

  • Balance and mobility training

  • Guidance on safe movement and fall prevention

  • Home environment assessments and practical recommendations

  • Ongoing support and progress reviews

It’s a supportive, collaborative process — focused on helping you live more confidently and independently every day.

How to Book a Home Visit

Because our home visit service involves planning travel between different locations, we don’t offer online booking for these appointments. Instead, we arrange visits personally to ensure your session fits smoothly into the diary.

To book, simply call, email, text or WhatsApp — whatever’s easiest for you. We’ll chat through your needs, answer any questions, and arrange a convenient time for your first visit.

We’re here to make the process simple and supportive from the very first contact.
